Student Leave of Absence Request Form
When a pupil undertakes an activity away from school, including one which is not commissioned by the school but which occurs during school operating times, the school must be satisfied that appropriate arrangements are in place to safeguard the pupil. These arrangements must be accepted by the school (BGS) prior to the pupil starting the activity and prior to leave of absence being granted.
To enable BGS to fulfil its statutory requirements, parents and careers making a request for leave of absence must forward a copy of this form to the responsible organisation for completion. The responsible organisation must then return the form to BGS directly from an official email address. The email address will be used to confirm the communication trail.
